How to Get a Medical Marijuana Card in Illinois

Legal Requirements:

  1. Being an Illinois resident with proof of residency.
  2. Consult a doctor about using cannabis to treat one of the state’s qualifying medical conditions. If you’re eligible, the doctor will complete a Physician Written Certification Form. They’ll include this with the online application.
